Pet Grooming Safety Release Agreement:

Because we care about your pet’s safety and well-being, we want to assure you that every effort will be made to make your pet’s visit as pleasant and stress-free as possible. Due to the intimate nature of the grooming process, or a pet’s tolerance level for stress, strangers, or handling, some pets can become anxious or reactive during the grooming service.

Occasionally grooming can expose a hidden medical problem or aggravate a current one. This can occur during or after grooming.

If we determine that continuing the grooming service would present a threat to your pet’s emotional or physical health, and/or the safety of the groomer completing the service, we will discontinue the service and contact you immediately.

In the best interest of your pet, we request your permission to obtain immediate veterinary treatment should it become necessary.

Senior Pets: Also, realizing that aged pets have a greater chance of injury during grooming, I will not hold the grooming establishment responsible for accident or injury to my pet.

I understand and agree that if the grooming establishment deems it unsafe or unhealthy to continue for the pet or the groomer, the grooming establishment will end that day’s service and I will be responsible for any services rendered.

I hereby grant permission to this grooming establishment:

To utilize proper restraints for the health and safety of my pet and my grooming service provider.

To obtain emergency veterinary treatment for my pet at my expense.

Matting Disclaimer: I understand that if my pet is matted that there is higher risk of having razor irritation, nicks/cuts, and stress/trauma. I also understand that matting may have to be cut completely out as brushing doesn’t always remove matting.

Double Coat disclaimer: I understand that double coated dogs should not be shaved. This can cause their coat to not grow back which could become a permanent condition of your dog’s coat and could expose your dog’s sensitive skin to environmental hazards which could lead to conditions needing veterinary care, of which the groomer will not be responsible.

Section 2

Waiver & Release of Liability:

THIS AGREEMENT is entered into by and between Ruffstic Pet Care and the Pet Owner, with the following terms:
 

1. Owner authorizes Ruffstic Pet Care to take all necessary actions, at the sole discretion of Ruffstic Pet Care staff and management, to maintain the health and well-being of my pet(s), and furthermore agree to be responsible for any and all expenses relating to the same, including all ancillary services requested.

2. Owner understands that Ruffstic Pet Care has relied upon their representation that their dog is in good health at the time of check in for services at our facility. Any pre-existing health concerns or known allergies will be disclosed and discussed prior to check in. Furthermore, owner understands that we do not have a licensed veterinarian on site and may not be able to detect or diagnose a medical condition in your pet.

3. Owner specifically represents to Ruffstic Pet Care, that to their knowledge their pet has not been exposed to any contagious diseases within a thirty-day period prior to check in for boarding or any services at our facility. Owner further agrees to provide proof of, and maintain required (Rabies, Distemper, Bordetella) current vaccinations for their pet(s). These vaccinations need to be completed at least 2 weeks prior to their boarding appointment.

4. Owner understands that if their pet becomes ill or injured, or if the state of the pet’s health otherwise requires professional attention, Ruffstic Pet Care, in a non-emergency situation will attempt to contact and inform the owner prior to seeking medical attention. If no contact can be made, or in an emergency situation, Ruffstic Pet Care at its sole discretion, may enlist the services of a licensed veterinarian for treatment and thereafter attempt contact with owner. Furthermore, all expenses incurred from this treatment will be the sole responsibility of the owner.

5. Owner understands that senior pets may require a health check-up, to be completed by their veterinarian prior to check in, in order to verify that the pet is stable and free of health concerns that would present a risk while boarding in a non-medical kennel environment. Furthermore, the owner acknowledges the risks associated with allowing a senior dog to participate in boarding or grooming services, even with the written consent of a veterinarian.

6. Ruffstic Pet Care and its employees shall exercise reasonable care for pets under our supervision. Owner agrees to be solely and financially responsible for any and all of their pet’s behavior while it is in our care. This includes but is not limited to damages or injury to the pet itself, other pets, a staff member or any damages to the facility caused by the pet, unless incurred as a direct result of gross negligence on the part of Ruffsitc Pet Care or its staff members.

7. Owner understands that Ruffstic Pet Care is not responsible for loss, damage, or destruction of any items left with your pet.

8. Owner understands that by allowing their dog to participate in services offered by Ruffstic Pet Care, they hereby agree to allow Ruffstic Pet Care to take photographs or use images of their pet in print or otherwise for publication, promotion, or social media. Owner understands that video cameras (recording) are in use at Ruffstic Pet Care facility.

9. Ruffstic Pet Care reserves the right to refuse admittance to any dog and/or dismiss any dog that does not meet or maintain the health, temperament or other daycare/boarding standards. This determination shall be made at the sole discretion of Ruffstic Pet Care.

10. Owner understands that if their pet is not picked up on or before the scheduled check out date, they hereby authorize Ruffstic Pet Care to take whatever action is deemed necessary for continued care of their dog. Owner will be responsible for the cost of any such continuing care upon demand by Ruffstic Pet Care and understands that if they do not pick up their pet.

11. Owner agrees that if Owner’s dog(s) is/are not picked up by closing time, then Owner hereby authorizes overnight boarding appropriate for dog(s) and will be financially responsible for applicable overnight boarding charge.

12. Owner further understands and expressly agrees that each and every of the foregoing provisions contained above shall be in force and effect and shall apply to each and every occasion on which Owner’s dog(s) stays with Ruffstic Pet Care for daycare, extended boarding, or other services, as the case may be. This Agreement shall remain in full force and effect as between the parties for a period of 12 months unless otherwise cancelled or superseded by a writing signed by the parties.

 

Owner hereby certifies that Owner has read and understands this Waiver and Release of Liability and the regulations set forth above. By signing this agreement, Owner agrees to be bound by its terms and conditions.
